When requesting changes to your Local Education Agency (LEA) or attendance center(s), please complete the Request for Local Education Agency (LEA) or Attendance Center Changes.

DESE will begin accepting changes for the next school year in February.

Before changes are made to the systems, this form must be completed and APPROVED.  

Your LEA may want to consider the following when making changes: data reporting, Annual Performance Report (APR), resource/process reports, teacher loan forgiveness, and your LEA's ESEA Consolidated Application.
